Dynamically add footnotes to a weblog entry.

This plugin can be used anywhere on the page, regardless of whether the content is from one entry, two, or static HTML.  Any @{ref1}@ will automatically link to any @{fn1}{/fn1}@ on the same page, and they can be within different instances of the footnotes plugin tag.
